N2  -  Summary. WHO analgesic pain ladder and weak opioids. The WHO three-step analgesic ladder has been strongly questioned in the last few years. A recent trial showed that in patients with moderate pain, morphine should be used instead of weak opioids because more effective and with similar good tolerability. Although weak opioids are often used to treat severe pain, there is no evidence that they are less risky than morphine. All can have the same dose dependent adverse effects as morphine and require the same vigilance as morphine. A summary of the trial and evidences on the effects of weak opioids compared to morphine, paracetamol and NSAIDs are presented.
