Where is and where is psychopharmacology going: remembering the 40th anniversary of the 180 law. In coincidence with the 40th anniversary of the Italian reform of psychiatric care, the article, based on an articulated review of the most recent international literature on the epidemiology, therapeutic strategies, broader public health issues related to mental health care, proposes a synthetic evaluation of the present priorities, failures, perspectives. The pharrnacological component of interventions is still the protagonist, despite the increasing evidences on its very limited efficacy and documented toxicity, and the documented absence over the last 15 years of new drugs. Contrary to the repeated recommendations provided by authorities and experts in favour of a serious and responsible recognition of the unacceptable insufficient and still decreasing investment in human (not only nor principally medical) resources, the literature documents an amazing scarcity of field experiences targeted to experiment innovative collaborative team and community based strategies with long enough follow-up with hard outcomes which include better recovery and personal autonomy. It is suggested that the present situation should become a priority specifically for research on the nurses role in this critical area of care, where fundamental human rights could be protected only through a comprehensive strategy of care.
